Door fees just explicitly break out the spa's share; expect to tip your MA too, and expect the door fee + tip to be about the same as when you pay all in one. Also, not all spas have separate door fees; it makes no difference to the total cost. I've never been anywhere without having a private room, and the better spas now have showers in the rooms so you never have to leave it at all.
